Adding and Using Category Forms in Registration

PheedLoop’s registration allows for a variety of data-retrieving features including the option to create custom forms with logic-based questions. Forms assigned to a category will appear on the Details page during registration.

Creating Category Forms

  1. From the Event Dashboard, navigate to Advanced > Custom Forms
  2. Above the Custom Forms table, select Create
  3. Under Name, fill out the name of the custom form
  4. Under Category, select General Survey
  5. Select Add Question and choose the type of question you want to fill out
    1. Toggle on the Required checkbox to make the question required for attendees.
    2. Under Name, fill out the question.
    3. Depending on the type of question you choose, will determine the steps for question creation.
  1. Select Save Changes

Adding Forms to Categories in Registration

  1. From the Event Dashboard, navigate to Registration > Categories
  2. Select a category you would like to add a custom form to from the list
    1. Or above the categories table, select Create.
  1. In the category editor form, navigate Basic Settings
  2. Under Connected Form, select the custom form associated with the category
  3. Select Save Changes

Downloading Category Form Reports

You can also include the form in the Registrations report as an alternative for convenience.

  1. From the Event Dashboard, navigate to Data & Reports > Custom Reports
  2. Above the Custom Reports table, select Create
  3. Under Report Type, select Registrations
  4. Under Fields, select Form, Attendee - Full Name, Attendee - Email, and any other fields that would be important for you to extract
  5. Under Registration Category, select the category you wish to retrieve results from
  6. Select Save Changes

Downloading the Custom Report

  1. From the Event Dashboard, navigate to Data & Reports > Custom Reports
  2. Select the Form Response or Registrations report you created
  3. Above the Custom Report table, select Actions > Generate Report
  4. Select Download Previous Report
    1. Depending on the size of the report you may need to refresh your browser for this button to appear beside the custom report in the list.
