Adding and Using Category Forms in Registration
PheedLoop’s registration allows for a variety of data-retrieving features including the option to create custom forms with logic-based questions. Forms assigned to a category will appear on the Details page during registration.

Creating Category Forms
- From the Event Dashboard, navigate to Advanced > Custom Forms
- Above the Custom Forms table, select Create
- Under Name, fill out the name of the custom form
- Under Category, select General Survey
- Select Add Question and choose the type of question you want to fill out
- Toggle on the Required checkbox to make the question required for attendees.
- Under Name, fill out the question.
- Depending on the type of question you choose, will determine the steps for question creation.
- Select Save Changes
Adding Forms to Categories in Registration
- From the Event Dashboard, navigate to Registration > Categories
- Select a category you would like to add a custom form to from the list
- Or above the categories table, select Create.
- In the category editor form, navigate Basic Settings
- Under Connected Form, select the custom form associated with the category
- Select Save Changes
Downloading Category Form Reports
You can also include the form in the Registrations report as an alternative for convenience.
- From the Event Dashboard, navigate to Data & Reports > Custom Reports
- Above the Custom Reports table, select Create
- Under Report Type, select Registrations
- Under Fields, select Form, Attendee - Full Name, Attendee - Email, and any other fields that would be important for you to extract
- Under Registration Category, select the category you wish to retrieve results from
- Select Save Changes
Downloading the Custom Report
- From the Event Dashboard, navigate to Data & Reports > Custom Reports
- Select the Form Response or Registrations report you created
- Above the Custom Report table, select Actions > Generate Report
- Select Download Previous Report
- Depending on the size of the report you may need to refresh your browser for this button to appear beside the custom report in the list.
