Collecting Custom Ticket Holder Information Using Ticket Forms

Many events have custom questions that may want to ask their Ticket Holders/Attendees. This article will walk you through the process of how to set-up your custom forms for individual tickets.

How to Create a Ticket Form

The first step to collecting custom information from your registrants is to create a Category Form with your questions listed. 


  1. From the Event Dashboard, navigate to Advanced > Custom Forms and select Create

  1. Under Name, fill out the name of the custom form

  1. Under Category, select General Survey

  1. Select Add Question and choose the type of question you want to fill out

    1. Toggle on the Required checkbox to make the question required for attendees.
    2. Under the Name field, fill out the name of the question.
    3. Under the Description field, enter any additional information for the registrant to know.
    4. Enter any choices for the registrant to choose from. For text based questions, the registrant will be able to enter their answer in a text box.

  1. Select Save Changes

How to Assign a Ticket Form to a Registration Category

Once your custom form is completed, you can assign the form to the desired registration category. As registrants go through the registration process, they will be prompted to complete the custom form. Required questions must be answered before they can continue with the registration process.

  1. From the Event Dashboard, navigate to Registration > Tickets and select a category you would like to add a custom form to from the list.

  1. In the ticket editor form, scroll down to Connections

  1. Under the Ticket Form field, select the custom form associated with the category

  1. Select Save Changes

Any registrants will have to complete this form for each Ticket Holder before being able to proceed to checkout.

Please note, once responses are submitted, removing a question from the custom form will also remove any response(s) for the deleted question. We recommend hiding a question instead of deleting it to preserve response data.

Gathering Form Responses as a report
