Collecting Custom Registrant Information Using Category Forms
Disclaimer: If you haven’t already done so, please review Registrants and Ticket Holders - What’s the Difference? to ensure that you are collecting information from the appropriate stakeholder. This article reviews how to collect custom information from Registrants. If you would like to collect custom information from Ticket Holders, please refer to our Collecting Custom Ticket Holder Information Using Ticket Forms article.
Many events have custom questions that may want to ask their registrants. This article will walk you through the process of how to set-up your custom questions in PheedLoop using Category Forms. If your event has specific questions to ask to each ticket holder, we recommend using our Ticket Form feature instead.

How to Create a Category Form
The first step to collecting custom information from your registrants is to create a Category Form with your questions listed.
- From the Event Dashboard, navigate to Advanced > Custom Forms and select Create
- Under Name, fill out the name of the custom form
- Under Category, select General Survey
- Select Add Question and choose the type of question you want to fill out
- Toggle on the Required checkbox to make the question required for attendees.
- Under the Name field, fill out the name of the question.
- Under the Description field, enter any additional information for the registrant to know.
- Enter any choices for the registrant to choose from. For text based questions, the registrant will be able to enter their answer in a text box.
- Select Save Changes
Assigning the Custom Form to a Registration Category
Once your custom form is completed, you can assign the form to the desired registration category. As registrants go through the registration process, they will be prompted to complete the custom form. Required questions must be answered before they can continue with the registration process.
- From the Event Dashboard, navigate to Registration > Categories and select a category you would like to add a custom form to from the list.
- In the category editor form, scroll down to Basic Settings
- Under the Connected Form field, select the custom form associated with the category
- Select Save Changes
Please note, once responses are submitted, removing a question from the custom form will also remove any response(s) for the deleted question. We recommend hiding a question instead of deleting it to preserve response data.
Gathering Form Responses as a report
Once all the responses have been submitted, they can be easily found and exported as a CSV file with PheedLoop’s Custom Report feature. For more information, please review our How to Download Registration Data: Registration, and Ticket Purchase Information article.
Conclusion
